If you have not already done so please read the Subcontractor Introductory Letter, then return a signed copy of the Subcontract along with a completed W9, and a Certificate of Insurance with Additional Insured and Waiver of Subrogation endorsements matching the Insurance Example document. Return all completed documents to dlaursen@rvemontrose.com
